Referral Packet Checklist

Gather the following documents and send them to Amy Anderson. Reference the Standards for Providers Manual (SFPM) Section 9.3.2 for more:

  1. SA Service Authorization
  2. VR1884 AT Services for Customers with Visual Impairments Referral
  3. EAS Employment Assistance Specialist Report or Basic Consultation Report
  4. VR2902 Baseline Skills Assessment (or include this item in SA if none exists)

Reminders and Tips

Gather the following documents and send them to Amy Anderson. Reference the Standards for Providers Manual (SFPM) Section 9.3.2 for more:

Vision – Visual acuity & eye condition

 

Location –  Facility, Customer Site, or Virtual training location preference (See our FAQs for more details)

 

Baseline – Baseline Assessment is included in the packet, or requested in the SA

 

Training Hours – Start with 3 or more hours on the initial SA for immediate skill-building

 

Post Assessment – Post-training assessment is requested on the SA (SFPM Section 9.5.2)

 

Due Date –  Please mark the due date 60 days from the start date (See our FAQs for more details)
